Medical Claims
If you want to view your claims online, you must create an account with Aetna by clicking on the View Claims Online link below. You can also file claims directly by completing the Medical and Prescription claim forms below.
Dental Claims
If you want to view your claims online, you must create an account with MetLife through metlife.com/mybenefits. You will need your MetLife Dental ID number available on your ID card. If you need your Member ID Number before you get your ID card, please contact MetLife Dental customer service at 1 (800) 942-0854. You may also order an ID card online through metlife.com/mybenefits. You can also file claims directly by completing the dental expense form below.
