In order to add SHIP coverage based on a qualifying event, review the below eligibility document and click the "enroll" button. Enrollment must be within 31 days of the qualifying event. You may be requested to provide documentation verifying the date of the qualifying event (e.g. birth certificate, marriage license, I-94 travel history, boarding pass, letter showing a loss of coverage). Once you submit your request, the charges for the premium will be added to your student account. Coverage will be effective beginning the first day following your loss of coverage. Installment plans are not available for dependents who are added via qualifying event outside of the open enrollment period.
